This form is NOT an order form. It is intended only to gather information for customizing an item. Once we approve the custom request details on this form, we will communicate the approval through email with information on agreed item and cost. We will also provide the buyer with a new listing if an item does not exist in our store. Buyer can then continue with purchasing the item once they get an approval of the customization.
